Disaster Provider Plans

The following template was prepared to help our Chapter members in developing their own practice disaster plans. It was developed to be used in conjunction with thePreparedness Checklist for Pediatric Practices that was published by the American Academy of Pediatrics that can be easily downloaded at the Children and Disaster website.

Every practice is different and modifications of the template can be made to fit the situation. The green highlighted portions are explanatory and present to assist in completing the template. These areas can be deleted in the final plan. The black areas are part of the plan and should remain. The red highlighted areas are to insert specific information for the plan.
