What is a Medical Home?

A medical home is the way care is provided to your child and your family, it is not a building of any kind. The medical home has a knowledgeable, caring doctor and care team chosen by a family to take care of a child’s health needs.
A medical home is a family-centered and trusting partnership between a child, a child’s family and the care team who looks after the child’s health and well-being within a community-based system.
A Medical Home….
- Is how care is provided to your child and your family.
- Is not a building, house or hospital.
- Knows your child.
- Makes sure your child gets the right services for their total well-being.
- Is a partnership between your child, your family and the care team (doctors, nurses, health educators, staff, etc).
A medical home means your care team…
- Knows the health history of your child.
- Listens to your concerns and needs, as well as those of your child.
- Follows up with any other health care providers your child receives care from, when necessary.
- Works in partnership with you to make sure that the medical and non-medical needs of your child and family are met.
- Creates a trusting relationship and works together with you and your child.
- Treats your child with compassion and an understanding of his/her strengths.
- Develops a care plan with you and your child when needed.
- Respects and honors your culture and traditions.
